We are very excited to begin your pain-free journey with you! Here are some things you may want to know for your first visit with us.
Arrive Early

Please arrive 10 – 15 minutes prior to your scheduled appointment time to complete any other registration forms and to allow time to verify your insurance benefits (if applicable). Your prompt arrival helps us make sure we have any other additional forms required by insurance completed and helps our physicians stay on schedule. You do have the option of having the appropriate paperwork emailed to you prior to your first visit and giving your insurance information over the phone to help you save time as well.
How long will my appointment take?

Allow 45 – 60 minutes for your first visit for our team to complete a thorough comprehensive exam on your initial visit and at any new injury examination.
What should I wear?

Thought there is no specific dress requirement, most patients dress in comfortable clothing, such as shorts and t-shirt or workout clothing that allows free movement. You may also bring a change of clothing with you if prefer.
What do I need to bring with me?

Please bring any applicable imaging reports you have for recent X-Ray, MRI or CT scans. If you do not have a copy of the imaging report, a Medical Records Release form will be available for you at your first visit to sign, or this can be emailed to you prior to your visit.

A PHOTO ID and VALID INSURANCE card if you wish to bill insurance.

A copy of your ACTIVE MEDICATIONS.
Do I need to have x-rays?

Dr. Jimenez will perform a detailed history and examination at the time of your first appointment. Should there be anything in the history and examination that may warrant an x-ray to be taken, then it will be done at that time in the office for patient convenience. If Dr. Jimenez does not feel that it is appropriate to take imaging based on those findings, then an x-ray will not be taken.
How soon can I be seen?

We will always work to get you seen by a physician as soon as possible, same day if necessary. Appointments requested at peak times (5pm-7pm and Saturday mornings) may have a waiting period as these are our most popular appointment times. It’s important to us for the physician to have the time available to complete a thorough evaluation of your case.
